Excused Absence/Late Arrival/Early Departure Request Form

This form is to request a one-time absence or late arrival NOT associated with an academic conflict. Not all absences/tardy requests will be approved, and students are responsible for making up for any rehearsal time they’ve missed. For recurring academic course conflicts, please complete the “Class Conflict Form.”
